All card orders are subject to the following lead time from the date this proof is approved and submitted.
4-6 weeks for personalized cards & 8-10 weeks for customized cards. Lead times are subject to change based on current demand.

Cards will not be printed until this form is approved and received by CCI.

It is the responsibility of the customer to thoroughly check approval samples before signing final approval. Card Concepts, Inc. cannot be held liable for errors / omissions not stated in writing by the customer. Once approved and sent in to CCI, no changes can be made to the card artwork unless stated on this form.

CCI will attempt to follow up on this approval if not received immediately. The customer is responsible for ensuring that CCI receives this form.
